This is the very thing I am trying to decide if I should do or not, because sometimes I feel the word needs to be emphasized more than italics implies, but wasn't sure if caps was suitable for literature. I'm not sure how I feel about it in a manuscript, though. In your example above, the all caps feels redundant with the dialogue tag. It also feels like broadsword rather than a scalpel ... which appears to be that character's intent!
